Job summary

FOX Rehabilitation is growing in Red Bank, NJ, and is seeking licensed Physical Therapists to deliver one-on-one geriatric care in homes and senior living communities. The role offers autonomy, flexible scheduling, and meaningful patient outcomes through the Geriatric House Calls model.

Telehealth can be incorporated to extend access, with opportunities to collaborate in an interdisciplinary team and grow professionally within a supportive clinic culture.

Qualifications

• Valid Physical Therapy license in the state(s) of practice or eligibility to obtain
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
• Document effectively within FOX’s EMR system in accordance with regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
• Provide telehealth / virtual care when appropriate to extend access and flexibility
