Job summary

VITAS Healthcare is seeking a Home Care Registered Nurse (RN) to ensure hospice patients are comfortable and to build compassionate, meaningful connections with their families. The role emphasizes patient-centered care with autonomy at the bedside and a focus on comfort-focused end-of-life care.

The RN will work primarily in patients' homes, visiting 4–6 patients daily, and will participate in weekly interdisciplinary team meetings to coordinate holistic support.

Qualifications

• Current Illinois nursing license.
• Minimum of two years of nursing experience in hospice, home health, or community health in the last five years.
• Reliable transportation.
• Commitment to compassionate end-of-life care.

Responsibilities

• Provide comfort and compassionate care to hospice patients in their homes.
• Autonomously make care decisions at the bedside to relieve suffering.
• Educate patients and families about palliative care and options.
• Collaborate weekly with an interdisciplinary team at the local program office.
