Job summary

Corewell Health West, Visiting Nurses Team, is seeking experienced RNs, LPNs, and new grads for home care in the Grand Rapids area. Nurses will visit 5–7 patients per shift in home settings, applying clinical judgment to implement treatment plans.

The role supports autonomous practice, collaboration with an interdisciplinary team, and a strong emphasis on patient-centered care in the home environment. A sign-on bonus is offered and benefits are provided.

Qualifications

• Associates in Nursing required.
• Bachelor’s in Nursing required within 5 years.
• MI Registered Nurse (RN) required.
• MI Driver’s License required.
• AHA or ARC Basic Life Support (BLS) required within 90 days.

Responsibilities

• Access and implement the treatment plan under the Nursing Manager guidance.
• Assess, monitor, plan and take action to prevent physiological complications.
• Collaborate with interdisciplinary team to ensure high-quality home health care.
