Job summary

Corewell Health West Visiting Nurses Team seeks experienced RN/LPNs to provide autonomous home health care across West-Central Michigan. You will assess, monitor, and implement treatment plans for patients in their own homes, visiting 5–7 patients per shift.

We welcome new grads and emphasize critical thinking, teamwork, and high-quality care. Eligible for a sign-on bonus up to $10,000 with a comprehensive benefits package and day shift schedule, 40 hours weekly.

Qualifications

• Current RN or LPN licensure in Michigan.
• Experience in home health is preferred, new grads welcome.
• Ability to work autonomously in patients' homes.

Responsibilities

• Assess and implement treatment plans under Nursing Manager in home health.
• Monitor patients and adjust care to prevent complications.
• Collaborate with interdisciplinary team to support patient health.
