Job summary

VitalCaring seeks a Field-Based Registered Nurse (RN) to lead care for patients in home health. You will assess, plan, and deliver advanced nursing interventions, coordinating with physicians and care teams to improve outcomes within patients' homes.

Qualifications

• RN license and CPR certification required.
• Graduate of an accredited nursing program.
• Minimum one year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred).
• Strong clinical judgment and ability to think independently.
• Excellent communication skills with patient-centered approach.
• Ability to manage a caseload and prioritize effectively; proficient in EMR.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care.
• Develop and manage individualized care plans in collaboration with physicians and care teams.
• Deliver skilled nursing care, including medication management, treatments, and interventions.
• Serve as case manager, coordinating care across therapists, aides, and physicians.
• Monitor patient progress and make clinical decisions based on changing conditions.
• Educate patients and families to support disease management and independence.
• Coordinate transitions and discharge planning to reduce readmissions.
• Maintain timely, accurate documentation in the EMR.
• Supervise and support Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
