Job summary

VitalCaring is seeking a Registered Nurse (RN) for field-based Home Health to lead care in patients' homes. You will assess, plan, and implement skilled nursing care, coordinating with physicians and interdisciplinary teams to optimize outcomes.

Qualifications

• Active RN license (or compact eligibility) and CPR certification.
• Graduate of an accredited nursing program.
• Minimum one year RN experience; Home Health or related settings preferred.
• Strong clinical judgment and independent thinking.
• Excellent communication and patient-centered approach.
• Proficiency with EMR systems and documentation.
• Valid driver's license and reliable transportation.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care
• Develop and manage individualized care plans with physicians and care teams
• Deliver skilled nursing care, including medication management and treatments
• Serve as case manager coordinating across therapists, aides, and physicians
• Monitor progress and make clinical decisions based on changes
• Educate patients and families for disease management and independence
• Coordinate care transitions and discharge planning to reduce readmissions
• Maintain timely documentation in the EMR
• Supervise Home Health Aides as needed
• Participate in on-call rotation
