VitalCaring Group seeks an experienced Home Health RN to lead patient care in the home setting. You will manage plans of care, coordinate across disciplines, and drive outcomes from admission through discharge.

You’ll combine advanced assessment, care planning, and patient education to reduce hospitalizations and support long-term independence at home. Join a supportive team with strong clinical leadership.

Qualifikationen

• Must hold an active RN license (or compact eligibility).
• Minimum 1 year RN experience; home health or acute care preferred.
• Strong clinical judgment and ability to think independently; excellent communication.

Aufgaben

• Conduct comprehensive in-home assessments to evaluate clinical status and goals of care.
• Develop and manage individualized care plans with physicians and care teams.
• Deliver skilled nursing care including medication management and treatments.
• Coordinate care as case manager across therapists, aides, and physicians.
• Monitor patient progress and adjust care plans as conditions change.
• Educate patients and families to support disease management and independence.
• Coordinate care transitions and discharge planning to reduce readmissions.
• Maintain timely documentation in the EMR.
• Supervise Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
