Qualifications

• Current NH nursing license in good standing.
• Experience in home health or similar settings preferred.
• Ability to assess and respond to changes in patient condition.

Responsibilities

• Provide skilled nursing and rehabilitative care in patients' homes.
• Perform comprehensive assessments and document findings.
• Triage urgent clinical situations and coordinate care with families.
• Manage medications, wound care, IV therapy, and post-surgical needs.
