BAYADA Home Health Care is seeking a Registered Nurse (RN) for home health visits in Litchfield, New Hampshire. Qualifications

• NH RN license in good standing
• Graduate from an accredited nursing program as indicated by transcript or diploma
• At least 1 year of adult/geriatric care experience in acute, emergency, home health, hospice, ICU, med-surg, sub-acute or rehab settings

Responsibilities

• Perform comprehensive head-to-toe assessments and identify changes in patient condition
• Deliver skilled nursing care and rehabilitative services in the home setting
• Triage and manage urgent clinical situations independently in patients' homes
• Perform medication reconciliation, teaching, and safety monitoring
• Document wound assessments and care in the chart
