Banner Home Health in Greeley, Colorado seeks a compassionate Physical Therapist to join the home health team. You will perform evaluations, develop treatment plans, supervise PTAs, and manage a flexible schedule with a focus on patient-centered care.

Ideal candidates have 2+ years of PT experience or are recent graduates with strong clinical skills. We provide tools and support for independent practice in the patient home setting.

Qualifications

• Graduated from an APTA accredited school of Physical Therapy or equivalent.
• Must hold a current Physical Therapy license in the practicing state; BLS certification required.
• Ability to use computer software in automated patient care environments; proficient with typical PT tools.

Responsibilities

• Evaluate patients and develop plans of care with measurable goals.
• Provide skilled PT treatment and monitor progress; guide and educate patient and family.
• Document therapy sessions in accordance with guidelines and timelines.
• Communicate results and progress with physicians, nurses and the care team.
• Supervise PTAs, ATCs, and therapy staff as required.
• Participate in departmental meetings and process improvements.
