Job summary

CenterWell Home Health in Brooklyn Park, MN seeks a Home Health Registered Nurse to deliver one-on-one in-home nursing care. You will administer medications, perform wound care, and conduct patient assessments to support recovery and independence.

You will collaborate with physicians to implement individualized plans, monitor changes in condition, educate patients and families, maintain records, and supervise Home Health Aides in a patient-focused home care setting.

Qualifications

• Med surg, ICU, ER, or acute care experience.
• Current and unrestricted RN licensure.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide skilled nursing care to patients, working onsite in their homes, including administering medications and wound care.
• Develop and implement individualized care plans in collaboration with physicians and clinicians.
• Monitor patients' conditions and report changes to the physician and Clinical Manager.
• Educate patients and families on disease management, medications, and treatments.
• Maintain accurate patient care records and coordinate with other healthcare professionals.
• Report progress to the physician and Clinical Manager.
• Oversee Home Health Aides and ensure proper documentation.
