Job summary

OnSite Medical Solutions is seeking a compassionate Nurse Practitioner to perform consultations in nursing homes and assisted living facilities. The provider will deliver high-level medical and mental health care with administrative support and clinical mentorship in a flexible, autonomous practice.

Strong candidates hold an MSN with FNP/ANP/AGNP/ACNP credentials, SC licensure, and DEA registration, with 1+ year of experience and excellent communication and documentation skills.

Qualifications

• Master of Science in Nursing with FNP/ANP/AGNP/AGACNP/ACNP certification required, active SC license.
• DEA registration active and in good standing.
• Minimum 1 year of experience in a relevant clinical setting requested.

Responsibilities

• Provide consultations and manage acute, subacute, and primary care needs for older adults.
• Collaborate with nursing home staff, residents, and families to coordinate care.
• Educate patients and families on best practices and non-pharmacological approaches where appropriate.
