Qualifications

• Active RN license (or compact eligibility).
• Graduate from an accredited nursing program.
• Minimum 1 year of clinical nursing experience.
• Strong clinical judgment and compassionate care approach.
• Ability to manage a caseload independently.
• Excellent communication skills in sensitive situations.
• Basic EMR proficiency.
• Valid driver's license and reliable transportation.

Responsibilities

• Lead patient-centered hospice care by conducting assessments and developing comfort-focused plans.
• Act as care coordinator across the interdisciplinary team.
• Provide education, emotional support, and guidance to patients and families.
