Qualifications

• Graduate of an accredited Physician Assistant Program and NCCPA certified or Nurse Practitioner program and certification.
• Current licensure to practice in Texas as NP or PA, and Family Medicine credentialing.
• Current DEA and CPR certification.
• Valid Texas Driver’s License and credentialing requirements ability.
• Comfortable working as a solo provider within a clinic team and strong communication.

Responsibilities

• Interview members, perform exams, order tests, prescribe meds, and recommend treatments.
• Provide continuity for patients with long-term conditions within managed care.
• Make referrals to secondary/tertiary care with care team collaboration.
• Perform procedures and health maintenance visits, immunization reviews, and guidance.
• Document all contacts in electronic medical record and participate in care team meetings.
• Contribute to clinical committees and quality improvement initiatives.
