Job summary

SummitRidge Hospital in Athens, GA is seeking an experienced Outpatient Therapist to provide group, family, and individual therapy. You will complete social histories and treatment plans as part of a collaborative treatment team, coordinating transitions and discharge planning for patients in care.

The role requires strong clinical skills, crisis intervention capability, and excellent verbal communication. A Master’s degree in SW/ Psychology/ Counseling is required, with post-master’s experience

Qualifications

• Master’s degree required in SW/ Psychology/ Counseling.
• Strong clinical skills to address crisis situations and to conduct group, family, and individual therapy.
• Strong verbal and communication skills.

Responsibilities

• Provide clinical services via group, family and individual therapy.
• Complete social histories and treatment plans as part of the treatment team.
• Coordinate transition and discharge planning for patients as a service broker.
• Maintain positive relationships with service providers to support care transitions.
