Job summary

Hartford HealthCare at Home is seeking a Visiting Registered Nurse to provide patient-focused home health services across Connecticut. Nurses will perform comprehensive assessments, coordinate with interdisciplinary teams, and support patient education to promote self-management and independence in the home setting.

The role requires an active CT RN license and at least one year of RN experience; homecare experience is preferred.

Qualifications

• CT RN license required.
• 1 year of RN experience; homecare experience preferred.
• Valid driver's license and reliable transportation required.

Responsibilities
