CenterWell Home Health is seeking a Home Health Physical Therapist to deliver patient-centered therapy in patients' homes, under the supervision of the Clinical Manager. You will assess function, develop rehab plans, and guide a multidisciplinary team to restore independence and improve quality of life.

Responsibilities include documenting progress, coordinating with physicians and family, and ensuring adherence to the Plan of Treatment.

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Responsabilités

• Plan and administer prescribed physical therapy treatment for patients in their homes.
• Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement a rehabilitation program aligned with the physician's Plan of Treatment and patient goals.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.
