Job summary

CenterWell Home Health is seeking a Home Health Physical Therapist to help patients regain strength and independence in the comfort of their homes. You will plan, implement, and monitor therapy to maximize function, collaborating with a multidisciplinary team and reporting progress to physicians and families.

This full-time role requires a current PT license, CPR certification, and strong teamwork. The position offers competitive pay per visit ($59.00–$83.00) and a comprehensive benefits

Qualifications

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• One year of experience as a Physical Therapist.

Responsibilities

• Plan and administer prescribed physical therapy treatment for patients, working onsite in their homes.
• Assess and document patients’ physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement a rehabilitation program aligned with physician Plan of Treatment and patient goals.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit required documentation.
