Orthopedic At-Home Physical Therapist - Per Diem

JAG Physical Therapy is seeking a skilled Physical Therapist specializing in orthopedic rehabilitation in the home setting for patients recovering from total hip and knee replacement surgeries and other orthopedic conditions. Our mission is to help patients regain mobility, strength, function, and confidence at home, emphasizing physical recovery along with emotional and social benefits of community re-engagement. We seek a compassionate and motivated therapist to evaluate and treat patients during their plan of care at home, with potential transition to outpatient clinics for continuity. What You’ll Need:

• Bachelor’s or Master’s Degree required; Doctorate preferred
• Licensed Physical Therapist in practicing state
• Completed state CEU requirements (NY, NJ)
• Valid CPR certification
• Experience with EMR software preferred

What You’ll Do:

• Educate patients on safe mobility, transfers, and ambulation considering post-op precautions
• Assess patient safety for discharge readiness through transfers, ambulation, and stair negotiation
• Document patient care data following regulatory and payer guidelines, including evaluations, notes, and discharge plans
• Complete billing and maintain productivity standards
• Prepare for evaluations by reviewing history and diagnoses
• Administer tests and measures, interpret results, and develop diagnosis and care plans
• Establish and modify treatment plans collaboratively with the interdisciplinary team
• Educate patients and support systems
• Provide functional training including ADLs/IADLs, therapeutic activities, and community reintegration
• Manage caseload treatment plans and exercise prescriptions
• Ensure interventions are safe, effective, and compliant with JAG standards
• Use Prompt EMR for documentation
• Attend annual billing and coding seminars
• Coordinate evaluations with scheduling teams
