Position Summary:
The AVP, Healthmap at Home will position Healthmap Solutions as an innovative and best-in-class organization for providing primary care, care coordination and care management services to our members directly in their home. This role will focus on building and operating Healthmap Solutions’ home delivery model, which is designed to support high risk members in the home as an extension of their treating physician. The AVP, Healthmap at Home will develop and implement the strategic vision for our Healthmap at Home program as well as ensure the tactical & operational execution.
Responsibilities:
- Responsible for designing, developing the Healthmap at Homes program that is aligned with organizational goals for clinical, financial and experience outcomes across all Regional Pods Healthmap at Home primary care program
- Oversee all aspects of the company's home-based program, including clinical, provider and client facing activities and information
- Create processes and best practices to understand and assess clinical value propositions based upon organizational direction and long-term strategic vision
- Lead and mentor a multidisciplinary team, providing guidance, coaching and professional development opportunities to ensure employee engagement and company culture is embedded in our teams
- Build and manage operational functions including licensure, clinical documentation, coding, billing and quality controls that meet federal and state regulatory requirements
- Utilize and analyze data to identify barriers and/or opportunities to develop additional strategies to improve operational efficiencies and effectiveness to achieve KPIs and company goals
- Collaborate with physicians, payers, and regulatory bodies to establish contractual collaboration agreements
- Collaborate with Leadership on strategic planning, program development and operations management across the regions to ensure Healthmap at Home model is achieving targets and aligns with the organization’s vision, mission, values
- Set the objectives to be achieved with both provider and patient interactions with a focus on identifying, presenting, and closing gaps based on clinical objectives
- Perform other duties as assigned
Requirements:
- Bachelor’s degree in healthcare administration, Business Administration, or a related field (Master's degree preferred)
- 10+ years of end-to-end operational leadership experience that includes delivery of primary care through multiple modalities including telehealth and in home.
- 5+ years of operational experience with all regulatory aspects of the deliver model including risk adjustment, coding and billing
- Strong experience in building comparable healthcare entities, including knowledge of Medicare, Medicaid and compliance requirements
- Strong knowledge of regulatory and payer requirements, including collaboration agreements
