ProHealth Physicians, part of the Optum family, has an immediate opening for an Associate Patient Care Coordinator. The 40-hour/week full-time role supports the care team by greeting patients, checking in/out, scheduling appointments, and handling inquiries in the EHR.

Based at 6 Northwestern Dr, Bloomfield CT, this position may float to nearby practices to cover business needs. Pay is hourly and ranges from $16.00 to $29.00 per hour.

Qualifications

• High School Diploma or GED required.
• 1+ year receptionist or customer service experience.
• Beginner level of computer proficiency in Microsoft Office.
• Access to reliable transportation and valid US driver\'s license.

Responsibilities

• Greet and check in patients; check-out process.
• Answer incoming calls and document patient inquiries in the EHR.
• Conduct outreach to patients for appointments or follow-up.
• Schedule appointments using standard scheduling protocols.
• Manage administrative in-basket pools and work queues.
• Scan documents into the EHR per standard protocols.
• Protect patient confidentiality of medical, personal and financial information.
• Provide coverage at other ProHealth locations as needed.

ProHealth Physicianshas an immediate opening for a friendly, patient-focused and detail-orientedAssociate Patient Care Coordinatorto join our growing team. The position is full-time (40 hours/week) Monday - Friday.

This position will support the care team and its patients by greeting and checking in patients, scheduling appointments, answering telephone inquiries, collecting payments and maintaining demographic information. Additionally, they will routinely use the electronic health record (EHR) according to ProHealth policies and standard workflows.

May be asked to float to local practices in the region for coverage depending on business need.

Schedule:Monday - Friday 8-hour shift between 7:00am-6:00pm. One Saturday per month 9am-1pm

Location: 6 Northwestern Dr. Bloomfield CT

Primary Responsibilities:

• Conduct check-in and check-out process
• Answer incoming calls and use EHR to document details of patient inquiries
• Conduct outreach to patients requiring appointments or service follow-up
• Schedule appointments using standard scheduling protocols
• Manage administrative in-basket pools and work queues
• Scan documents into the EHR per standard protocols
• Protect patients' rights by maintaining confidentiality of medical, personal and financial information
• Provide coverage at other ProHealth locations on an ad hoc basis
• Perform other duties as assigned

Required Qualifications:

• High School Diploma/GED
• 1+ years of receptionist or customer service experience
• Beginner level of computer proficiency in Microsoft Office
• Access to reliable transportation and valid US driver's license

Preferred Qualifications:

• Experience working in a primary care setting
• Experience working with EPIC

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
