Job summary

Crossroads Hospice & Palliative Care is seeking an Associate Medical Director to provide medical leadership and oversight of personalized treatment plans. You will collaborate with primary care physicians and the interdisciplinary team to ensure compassionate, patient-centered care.

Responsibilities include certifying/recertifying terminal illness, coordinating care, and supporting the hospice program to maintain high-quality clinical performance.

Qualifications

• MD or DO licensed to practice in the state.
• Legally authorized to practice medicine in the state of practice.
• Assumes overall responsibility for the medical component of the palliative client care program and supports the hospice program.
• Be responsive to the medical community to ensure high-quality, appropriate, and efficient utilization of hospice services.
• Assist in investigations of important problems in patient care and clinical performance.
• Assist the Agency in the coordination of patient care between clinical staff and patients' attending physicians.
• Acts as the Physician Representative of the interdisciplinary group (IDG) and/or attending physician to certify and recertify for subsequent election periods that the patient is terminally ill.

Responsibilities

• Certify and recertify patients as terminally ill in coordination with IDG and attending physicians.
• Coordinate patient care between clinical staff and attending physicians to ensure appropriate services.
• Support the hospice program and assist investigations into clinical performance issues.
