Position Summary

This crucial role is a transitional position toward becoming a Center Clinical P&L leader (i.e., Senior Medical Director) in the organization. The incumbent Center Medical Director (CMD) will leverage prior leadership experience to expedite learning and deliver the ChenMed clinical model. The CMD may simultaneously complete PCP Black Belt Training and Master the Medical Director Essentials within a combined, accelerated training pathway.

Key Metrics and Competencies

The CMD will be subject to, and have center oversight for, metrics regularly assessed to help clinicians become successful and reach Partnership status: panel size, patient outcomes (admissions and hospital sick days), CG CAHPS and net promoter score (patient experience), HEDIS and clinical gaps closures, and medical cost‑effectiveness measures. Each primary care provider (PCP) will have goals for these metrics and will be expected to work toward them with their center and market leadership and care team.

Core Responsibilities
  • Accountability and oversight for outcomes: Demonstrate accountability for outcomes, strong clinical care, and cost‑effectiveness for each patient in their panel of at least 300 risk‑adjusted patients (75% partial panel). Build trusting relationships that influence patient outcomes and change behaviors.
  • Coaching for health: Act as a health coach, helping patients set short‑ and long‑term health goals, partnering with them to work toward those goals, and frequently following up on progress.
  • Simplifying for action: Simplify and prioritize so that behavior change is actionable for the patient and for leading the care team toward performance goals.
  • Leadership: Lead center Super Huddle (SH) and Transforming Care Meeting (TCM) weekly care meetings, support the Center Manager/Center General Manager, and progressively assume responsibilities of the Senior Medical Director to supervise, performance manage, and train PCPs.
  • Clinical oversight: Independently provide care and oversight of outcomes for patients with acute and chronic illnesses, taking full accountability for patient care and outcomes while seeking consultation from specialists when needed.
  • Hospital engagement: Engage with the hospitalist whenever one of their patients is in the hospital, regardless of affiliation.
  • Care coordination: Responsible for assessment, diagnosis, treatment, management, education, health promotion, and documentation for patients with acute and complex chronic health needs.
  • Team leadership: Lead the care team (care promoter, care facilitator, care coordinator) for patients who can come to the office, or engage the transitional care team and other resources for patients unable to attend the office.
  • Culture and recruitment: Drive a positive culture that supports a cohesive team, excellent results, and relationships, promote a best‑place‑to‑work culture measured by employee engagement scores, and assist in recruiting patients and providers.
  • Future leadership: Upon progress toward consistent primary care performance trending toward the ChenMed PCP Partner level, the CMD may increasingly assume the Senior Medical Director role, including managing, mentoring, and coaching PCPs, leading rounding, partnering with leadership to improve financial performance, and expanding center membership.

Pay Range

$257,939 - $368,485 Salary. The posted pay range represents the base hourly rate or base annual full‑time salary for this position. Final compensation will depend on experience, education, geographic location, and other relevant factors. This position may also be eligible for bonuses or commissions.
