Job summary

Heritage Grove Estates in Topeka, Kansas is seeking an Assisted Wellness Director to lead resident-centered care, coordinate services with clinical staff, and promote a safe, engaging living environment for residents.

You will help develop care plans, monitor wellbeing, and support daily living activities while collaborating with families and care teams to ensure high-quality services. This role offers opportunities for growth and a supportive workplace culture.

Qualifications

• High school diploma or equivalent required.
• Previous experience in healthcare, assisted living, or senior care preferred, but not required.
• Knowledge of senior care practices and resident support preferred.
• Strong communication, organization, and interpersonal skills.
• Ability to work collaboratively within a team environment.
• Current CPR/First Aid certification preferred.
• Ability to work flexible schedules, including weekends and holidays as needed.
• Commitment to providing compassionate, resident-centered care.

Responsibilities

• Collaborate with interdisciplinary team members to develop and implement individualized care plans based on resident needs and preferences.
• Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support as needed.
• Conduct routine wellness checks and observe for changes in resident condition, reporting concerns to the appropriate clinical staff.
• Maintain accurate and timely documentation, including care plans, progress notes, and vital signs in accordance with facility policies.
• Communicate effectively with residents, families, and care team members regarding updates and concerns.
• Support resident independence while ensuring safety and comfort.
• Participate in quality improvement initiatives to enhance resident care and service delivery.
• Assist with coordinating services and resources to support resident needs.

Position Overview (Assisted Wellness Director)

Heritage Grove Estates (Assisted Living Facility) is seeking a compassionate and organized Assisted Wellness Director to support the delivery of high-quality, resident-centered care. This role is essential in ensuring residents receive individualized attention, coordinated services, and support with daily living needs.

As the Assisted Wellness Director, you will work closely with clinical staff, residents, and families to help develop and implement care plans, monitor resident well-being, and promote a safe, supportive, and engaging environment.
