Bristol Glen, a UMC Community in Newton, NJ, is hiring a full-time Assisted Living Social Worker. The role supports residents and families, coordinates care planning with the interdisciplinary team, and ensures documentation is complete and up to date.

Qualifications include a BSW or related field and NJ CSW licensure, plus 1+ year of healthcare or long-term care experience. Shift options run 8:00 a.m.–4:30 p.m. or 8:30 a.m.–5:00 p.m.

Qualifications

• Bachelor’s degree in Social Work (BSW) or a Human Service field is required.
• Certified Social Worker (CSW) license with the State of New Jersey is required.
• 1+ year of healthcare or long‑term care experience.
• Proficiency in a clinical software system is required.
• Understanding of the MDS, care plan, and admission and discharge processes.

Responsabilités

• Build relationships with community agencies and resources to support resident and family needs during care transitions.
• Advocate for resident rights, well-being, and quality of life while supporting residents and families.
• Maintain social services documentation, including assessments, care plans, discharge records, and resident files.
• Identify, address, and report concerns related to resident safety, abuse, neglect, or mistreatment.
• Educate team members on resident rights, dignity, independence, and person-centered care practices.
• Coordinate communication with residents, families, and the interdisciplinary team regarding care planning and updates.

Formation

Bachelor’s degree in Social Work (BSW) or a Human Service field
