Job summary

Jasper of Meadows Place, a Sonida Senior Living community, in Meadows Place, TX, seeks an Assistant Wellness Director to lead nursing services and ensure quality resident care.

You will supervise care staff, implement policies, conduct assessments, and coordinate with families while maintaining compliance with state and federal regulations. This on-site role offers a comprehensive benefits package including health/dental insurance and retirement savings.

Qualifications

• Graduate of an accredited school of nursing.
• Two years of professional nursing experience in a senior living care setting.
• Two years of supervisory/management experience with direct care staff.
• Some states may allow for this to be an unlicensed position (WI, TX, SC).

Responsibilities

• Maintains and implements nursing policies and procedures that reflect current standards of nursing practice, company, and facility philosophy of care.
• Communicates and interprets policies and procedures to nursing staff; monitors practices for effective implementation and results.
• Conducts resident assessments, suggesting level of needed service packages to residents and families, as directed.
• Ensures delivery of compassionate quality care with adequate services and staff coverage, cleanliness, and appropriate facility conditions.
• Demonstrates knowledge of and application of Clinical Quality Indicators.
• Reviews 24-hour reports daily to ensure timely responses to significant changes in condition and incidents.

Assistant Wellness Director Responsibilities include:

• Maintains and implements nursing policies and procedures that reflect current standards of nursing practice, company, and facility philosophy of care and practices while maintaining compliance with state and federal laws and regulations.
• Communicates and interprets policies and procedures to nursing staff. Monitors practices for effective implementation and achievement of results.
• Conducts resident assessments, suggesting level of needed service packages to residents and their families, as directed.
• Ensures delivery of compassionate quality care evidenced by adequate services and staff coverage for the facility, appropriateness of staff, absence of odors, cleanliness, absence of pressure wounds, and apparent maintenance of optimal physical, mental and, psychosocial function.
• Demonstrates knowledge of and application of Key Clinical Quality Indicators.
• Reviews 24-hour report from every unit daily to ensure timely, effective responses to significant changes in condition, transfers, discharges, use of physical or chemical restraints, unexplained injuries, potential abuse or neglect, medication errors, loss of resident property, or any evidence of resident or family dissatisfaction, as directed.
• Listens and documents significant changes in condition of residents; including, but not limited to, incidents, unexplained injuries, medication errors, loss of resident property, or evidence of resident or family dissatisfaction per agency policies and state regulations.
