Genesis HealthCare in Westbrook, Maine, is seeking a dedicated social services professional to collaborate with patients and families, provide resource information, and support transitions within the nursing center. You will respond to family-identified issues and document care pathways under supervision of a qualified social worker or consultant.

The role requires an associate degree in human services and at least two years in long-term care; state-certification requirements must be met.

Posted Salary Range

USD $20.00 - USD $25.00 /Hr.
