Position: Assistant RN Clinical Manager – Hospice & Home Health

Location: Libertyville, IL – Lake County

Schedule: Full-time

Overview:

Integrated Home Healthcare, Palliative, and Hospice is seeking a dedicated Assistant RN Clinical Manager to help lead our hospice and home health teams. This role supports the Clinical Manager in overseeing clinical operations, ensuring quality patient care, regulatory compliance, and staff development in a compassionate, team-oriented environment. Must have the ability to perform patient visits with 24/7 availability as needed.

Key Responsibilities:

• Supervise and support nursing and field staff; participate in care coordination
• Assist with individualized care planning and direct patient visits
• Provide 24/7 availability for patient care needs and urgent clinical support
• Monitor patient outcomes and ensure timely, high-quality care delivery
• Ensure compliance with Medicare, state, and federal regulations
• Support onboarding, training, and evaluations of clinical staff
• Help manage scheduling, documentation accuracy, and clinical resources
• Serve as liaison among staff, patients, families, and providers

Qualifications:
