Responsibilities

• Teach independent living skills and assist with consumer personal care.
• Arrange/provide transportation for consumers.
• Assist consumers with recreation/inclusion and plan vacations.
• Administer, monitor and record medication distribution according to policy; review medical logs.
• Coordinate medical appointments and ensure documentation is accurate; attend appointments when needed.
• Ensure communication of medical recommendations and follow-up; complete in a timely fashion.
• Coordinate/provide staff training and provide information to staff to carry out duties.
• Communicate consumer issues/needs to all parties; guide staff on interactions with consumers, families and guardians.
• Address staff concerns; encourage idea sharing and improvements.
• Attend all staff and consumer-related meetings; ensure schedules are posted.
• Complete and submit required reports (daily, weekly, bi-weekly, monthly, quarterly, annual).
• Handle and document all financial transactions for consumer accounts; reconcile monthly.
• Ensure preventative maintenance of program vehicles; arrange regular service/inspections.
• Perform other duties as required by the supervisor.

PAY RATE

$21.15 per hour

JOB SUMMARY:

• Teach independent living skills and assist with/provide consumers with personal care.
• Arrange/provide transportation for consumers.

• Coordinate/provide all necessary agency and intra-program staff training; provide staff with all necessary information needed to effectively carry out their job duties.
• Ensure proper communication of all consumer issues/needs to all necessary parties; proactively teach/role model/guide staff on how to appropriately work with and interact with consumers, other staff, families, and guardians.
• Address staff concerns as needed; encourage staff to share ideas and suggestions.
• Attend all staff meetings, and other consumer-related meetings.
• Ensure staff, training, and beeper schedules are received and posted.
• Complete and submit all required daily, weekly, bi-weekly, monthly, quarterly, and annual reports as scheduled.

• Handle and document all financial transactions for consumer accounts in accordance with agency and DDD policy; reconcile all consumer accounts monthly.
• Ensure preventative maintenance of the program/vehicles(s) including regular vehicle service and inspections.
• Perform other duties as required by the nature of the position, or as requested by supervisor.
