Stillwater Post Acute is seeking a detail-oriented Assistant MDS Coordinator to support the MDS and clinical documentation process. This role works closely with the MDS Coordinator, nursing team, interdisciplinary colleagues, and other departments to ensure accurate, timely, and compliant documentation.

The ideal candidate is organized, knowledgeable about clinical documentation, and able to manage multiple deadlines while maintaining a strong focus on resident care and regulatory compliance.

Qualifications

  • Current LVN license.
  • Experience in skilled nursing, MDS, or clinical documentation preferred.
  • Knowledge of MDS processes and long-term care regulations preferred.
  • Strong organizational and time-management skills.
  • Excellent written and verbal communication skills.
  • Ability to work collaboratively with an interdisciplinary team.
  • Strong attention to detail and ability to meet deadlines.
  • Proficiency with electronic medical records and Microsoft Office or similar software.

Responsibilities

  • Assist the MDS Coordinator with the completion and coordination of MDS assessments.
  • Review clinical documentation and resident records for accuracy, completeness, and consistency.
  • Collaborate with nursing, rehabilitation, dietary, social services, and other members of the interdisciplinary team.
  • Assist with gathering information required for MDS assessments and care planning.
  • Monitor assessment schedules and help ensure required documentation is completed timely.
  • Identify documentation discrepancies and communicate concerns to the appropriate team members.
  • Assist with care plan development, updates, and follow-up as needed.
  • Maintain accurate records and support compliance with federal, state, and facility requirements.
  • Participate in interdisciplinary team meetings and quality assurance activities.
  • Protect resident confidentiality and follow HIPAA and facility policies.
  • Perform other duties related to MDS coordination and clinical documentation as assigned.
