Assistant Habilitation Supervisor - Bridges Program - St. Lawrence County

Position Type/Expected Hours of Work: Flexible to the needs of the program.

Qualifications: Associate Degree and one-year experience working with the developmentally disabled preferred; or a high school diploma or equivalency and two years’ experience working with the developmentally disabled required.

Relevant Certifications: Valid/acceptable NYS drivers’ license.

Work Authorization/Security Clearance: Authorized to work in the United States and pass all background clearances.

Summary/Objective:

  • Serve as the frontline supervisor to Direct Support Professional staff working in assigned program area.
  • Provide daily supervision and be responsible for the daily operation of the day program site, working under the direction and guidance of the Habilitation Program Coordinator.
  • Spend a majority of working hours in direct contact and interaction with Persons We Support.
  • Ensure timely and thorough completion of all paperwork and Therap documentation for Persons We Support in assigned classrooms.
  • Assist in the training and support of Direct Support Professional/Substitute Staff, under the direction/guidance of the Habilitation Program Coordinator.
  • Support the goals, objectives, and philosophy of the agency.
  • Work with measurable signs of PRIDE.

Essential Functions: Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • Participate in the implementation of goals, services, community outings, etc. with people receiving services in assigned classroom/site.
  • Assist the Habilitation Program Coordinator with the interview process for prospective DSP employees.
  • Recommend and be involved with all remedial assistance for assigned staff.
  • Intervene in instances of insubordination and deficiencies with job performance by conferring with employees and informing the Habilitation Program Coordinator.
  • Schedule meetings with all staff, minimally two times per month to discuss program issues and concerns.
  • Assist the Habilitation Program Coordinator with staff performance evaluations.
  • Complete agency management series.
  • Ensure compliance with Medicaid waiver standards, documentation procedures, CQL standards, and all other Arc Jefferson-St Lawrence policies.
  • Maintain positive working relationships with all members of the support team.
  • Report maintenance needs to the Habilitation Program Coordinator.
  • Request purchase of necessary program supplies.
  • Ensure implementation of ISP programs and staff services.
  • Participate in the completion of initial and updating of annual comprehensive functional assessments for People We Support.
  • May be required to administer medications as directed by physicians’ orders and agency policy.
  • Ensure proper storage for all classroom equipment and supplies.
  • Complete attendance in Therap.
  • Ensure all program staff is aware of pertinent information regarding people receiving services.
  • Monitor and review all personal funds for People We Support.
  • Ensure daily completion of the Day Habilitation daily assignment sheet.
  • Complete Fire Drill forms each time a drill is completed.
  • Ensure completion and accuracy of the Vehicle Mileage and Data form.
  • Complete the Protecting the Workplace checklist daily.
  • Promote a person-centered philosophy.
  • Participate with bus aide duty, as assigned.

Supervisory Responsibility: Serve as the frontline supervisor to Direct Support Professional staff.

Travel: As needed within assigned County.

Work Environment: A professional clinic where the people we support attend our program services on a daily basis.
