South Shore Care Center in Worthington, MN seeks an Assistant Director of Nursing to join our leadership team. This role blends clinical leadership with oversight of assessment and care planning to ensure high-quality resident care.

Responsibilities include supervising the nursing department, coordinating MDS processes, and leading quality and regulatory initiatives in a collaborative, resident-centered environment. Must align with policies and regulatory standards.

Qualifications

• Current RN license in Minnesota.
• Minimum of 2 years of nursing leadership experience in a long-term care or skilled nursing setting.
• Strong knowledge of MDS 3.0, care planning, and RAI guidelines.
• Excellent organizational, communication, and leadership skills.
• Ability to balance clinical oversight with administrative responsibilities.

• Work collaboratively with interdisciplinary team members to develop individualized resident care plans.
• Monitor documentation for accuracy and compliance with care plans, regulatory requirements, and facility policies.
• Educate and mentor nursing staff in MDS processes, care planning, and clinical best practices.
