Qualifications

• California Registered Pharmacist licensed by the state Board of Pharmacy.
• Board-certified or residency training in ambulatory care preferred.
• Active Basic Life Support (BLS) certification.
• PharmD degree from an accredited school of pharmacy recognized by the state board.

Responsibilities

• Provide clinical pharmaceutical care in ambulatory care using approved protocols.
• Monitor patients' drug therapies and assess outcomes for quality care.
• Collaborate with a multidisciplinary team to optimize patient safety and outcomes.
• Educate patients and providers to improve adherence and prescribing practices.
