The role reports to the Clinical Nurse Manager and supports day‑to‑day clinic leadership with a focus on value-based care.

Ideal candidates hold an active RN license (BSN preferred), have 2+ years of clinical nursing with supervisory experience, and can manage staffing, performance, and care transitions in a data‑driven

Qualifications

• RN license required; BSN preferred.
• Minimum 2 years nursing in a clinical setting with frail or elderly population.
• Minimum 1 year of supervisory experience mentoring and developing staff.
• Minimum 1 year of case management, care coordination, or related specialty preferred.
• Experience leading in a data-driven organization using reports to prioritize people and projects.

Responsibilities

• Provide leadership, oversight, and performance management to RN, LVN, and MA staff, ensuring training and role clarity.
• Oversee clinic function to address chronic and acute participant needs.
• Coordinate transitions with Complex Care team to meet post-discharge needs.
• Develop workforce plans and ensure appropriate staffing given diverse experience levels.
• Participate in direct participant care within RN level of supervision.
• Manage direct reports including hiring, training, supervising, and mentoring.
