Sangre de Cristo Community Care invites an experienced Registered Nurse to grow into clinical leadership as Assistant Clinical Manager. You’ll partner with the Clinical Manager to support Hospice and Home Health teams while making a meaningful impact on patients and families.

Ideal candidates are RN Case Managers or clinical leaders seeking to expand leadership skills in a collaborative, mission-driven setting with a reduced caseload and leadership responsibilities.

Qualifications

• Current Colorado RN license required.
• Minimum of three years of Hospice and/or Home Health experience.
• Strong clinical assessment and critical thinking skills.
• Excellent communication, organizational, and leadership abilities.
• Previous supervisory/leadership experience preferred; Medicare CoP knowledge preferred.
• Current CPR certification.
• Valid driver’s license with reliable transportation.

Responsibilities

• Partner with the Clinical Manager in daily oversight of Hospice and Home Health clinical operations.
• Provide clinical guidance, mentoring, and support to nurses and interdisciplinary team members.
• Assist with staff orientation, onboarding, and ongoing competency development.
• Coordinate clinician schedules and patient assignments to ensure quality care and efficient operations.
• Participate in interdisciplinary team meetings and case conferences.
• Monitor documentation quality and regulatory compliance; lead quality improvement initiatives.
• Conduct chart audits and support continuous improvement efforts.
• Serve as a resource for complex clinical situations and patient care concerns.
• Maintain a reduced patient caseload while supporting leadership duties.
• Assume delegated leadership responsibilities in the absence of the Clinical Manager.

As our Assistant Clinical Manager, you'll partner with the Clinical Manager to support our Hospice and Home Health teams while continuing to make a meaningful impact with patients and families.

What You'll Do

Requirements

• Current Colorado Registered Nurse (RN) license
• Minimum of three years of Hospice and/or Home Health experience
• Strong clinical assessment and critical thinking skills
• Excellent communication, organizational, and leadership abilities
• Previous supervisory, charge nurse, preceptor, or leadership experience preferred
• Knowledge of Medicare Conditions of Participation and regulatory requirements preferred
• Current CPR certification
• Valid driver's license, reliable transportation, and proof of automobile insurance
