Job description

Assessment Specialist - Flex Pool - Scioto and surrounding counties

CareStar, Inc. is seeking a Community Health Worker in the Clinical department to conduct program assessments using established tools and processes. This home-based position requires independent management of a caseload.

Position Details

Flex staff are employed on an "as needed" basis, working up to a maximum of 29 hours per week when services are required. These employees are generally not eligible for benefits unless specified in the Employee Benefit Eligibility Policy but are eligible for mileage reimbursement per CareStar policy.

Responsibilities and Qualifications

  • Active license as RN, LSW, or LISW is preferred.
  • At least 12 months of experience in home and community-based services within the past 5 years.
  • Knowledge of Medicare, Medicaid, home health care, and Home and Community-Based waiver programs is desirable.
  • Adherence to CareStar's rules and policies is required.
  • Maintain confidentiality of patient information; unlawful disclosures will be treated as disciplinary offenses.
  • Follow the Acceptable Use Policy for all CareStar information systems.

Location

This position is based in Scioto County and covers surrounding counties.
