Role Overview

In this Field RN role, you will be a vital part of an Integrated Care Model, relaying information about member needs and advocating for them. If located in New York City, NY, you will have the flexibility to work remotely, with some in-person visits to members' homes or community settings.

Primary Responsibilities
  1. Assess, plan, and implement individualized care strategies focusing on the least restrictive level of care.
  2. Utilize the NYS UAS Assessment in home or telehealth settings to develop person-centered service plans.
  3. Identify and refer social service programs, including financial, psychosocial, community, and state supportive services.
  4. Manage care plans throughout the continuum as a single point of contact.
  5. Communicate health-related information effectively to ensure coordinated, quality care.
  6. Advocate for patients and families to ensure their needs and choices are supported by the healthcare team.

Required Qualifications
  • Current, unrestricted Registered Nurse license
  • At least 2 years of relevant clinical experience
  • Over 1 year of community case management experience
  • Knowledge of home health, hospice, public health, or assisted living
  • Proficiency in MS Word, Excel, Outlook
  • Ability to travel within the assigned region
  • NY state ID or the ability to obtain one prior to hire
Preferred Qualifications
  • Behavioral health or clinical degree
  • Experience with electronic charting and community resource coordination
  • Field-based work experience
  • Background in managing complex medical or behavioral populations
  • Proficiency with UASNY

Compensation and Additional Information
