Job Summary:

As an Assessment & Care Planning Nurse (RN), you will complete virtual assessments with clients using tools such as Zoom or FaceTime. The primary goal of the assessment is to document in descriptive form how the individual is functioning in the environment in which they live; this includes documenting historical information related to the client’s self-reported medical history. Additionally, the Assessor role will observe and guide each client through demonstrations of ADL’s and cognitive screening exercises as outlined in the assessment tool by each insurance partnered with THB.

This role also encompasses QA of completed assessments for the expected compliance with quality, completeness, and carrier contract specifications. You will focus on accuracy and efficiency. You will be focused on observing and reporting the current status of the individual. You must remain objective.

Supervisory Responsibilities:

None

Duties/Responsibilities:

  • Evaluate an individual’s cognitive and functional status.
  • Comply with all assessment instructions and accurately complete the digital assessment documentation.
  • Submit the assessment within 24 hours of assessment.
  • Respond and make corrections, within 24 hours to any follow-up and clarification questions from your review team regarding each submitted assessment.
  • Communicate with Scheduling department, review & other clinical in a timely manner.
  • Describe current caregiver services.
  • Perform facility evaluations by completing a questionnaire with the facility administrator and collecting required documents.
  • Self-manage calendar and availability.
  • Quality assurance of Benefit Eligibility Assessments.
  • Maintain up-to-date knowledge of respective carrier specifications.
  • Completion of the POC/CIC documents.
  • Other duties as assigned/necessary.

Performance Metrics:

  • Accuracy Rate: Accuracy of assessment documentation.
  • Assessment Completion Time: Average time taken to complete assessments.
  • Response Time to Follow-up Queries: Time taken to respond to follow-up questions or clarifications from the review team regarding submitted assessments.

Required Skills/Abilities:

  • Ability to thrive in a fast-paced environment.

  • Excellent time management skills with a proven ability to meet deadlines.
  • Must have reliable internet capable of completing Zoom or FaceTime video calls.
  • Computer literacy with tools such as Zoom, FaceTime, Microsoft Exchange, etc.
  • Typing literacy (50+ WPM preferred).
  • Strong passion for helping others and problem-solving.
  • Must be 18 years of age or older.

Education and Experience:

  • Unencumbered RN license required in applicable state(s).
  • 5+ years as an RN-familiarity with a broad range of medications and conditions common in the geriatric population.
  • 6+ Months conducting Benefit Eligibility Assessments OR prior experience in a similar QA role.

Physical Requirements:

  • Ability to remain at your designated workstation for the duration of the workday.
  • Constantly operates a computer and other office productivity machinery, such as a phone and Voice over Internet Protocol (VoIP).
  • The ability to communicate information and ideas so others will understand. Must be able to exchange accurate information in these situations.
  • The ability to observe details at close range (typically on a computer screen).
  • This position offers the flexibility of remote work within the United States. Candidates must have a reliable internet connection and a designated work environment conducive to professional phone calls and sensitive data.
