Job Summary:
As an Assessment & Care Planning Nurse (RN), you will complete virtual assessments with clients using tools such as Zoom or FaceTime. The primary goal of the assessment is to document in descriptive form how the individual is functioning in the environment in which they live; this includes documenting historical information related to the client’s self-reported medical history. Additionally, the Assessor role will observe and guide each client through demonstrations of ADL’s and cognitive screening exercises as outlined in the assessment tool by each insurance partnered with THB.
This role also encompasses QA of completed assessments for the expected compliance with quality, completeness, and carrier contract specifications. You will focus on accuracy and efficiency. You will be focused on observing and reporting the current status of the individual. You must remain objective.
