The Care Manager assists individuals in maintaining an interactive process of informed decision-making about Long-Term Services and Supports. Assessment Care Manager While this position requires the ability to perform all of key responsibilities listed for a Care Manager, the primary responsibilities will focus on performing initial assessments and reassessments (Reassessments are for Community First Choice Assessors only) to establish eligibility for various contracted programs.
