As part of our (FACT)Flexible Assertive Community TreatmentTeam, you’ll play a vital role in providing peer and wellness services to our clients in their home and or in a community setting.

Some Key Duties:

  • Provide integrated, recovery-focused services that address physical, behavioral, and social health needs.
  • Demonstrate clinical expertise in peer recovery and wellness coach practices, offering support for clients with complex needs.
  • Serve as a liaison between inpatient and outpatient providers, assisting with discharge planning and aftercare coordination.
  • Promote health behavior change through evidence-based interventions, including health education, lifestyle coaching, and substance use prevention.
  • Facilitate peer support groups and one-on-one coaching, using tools like the Wellness Recovery Action Plan (WRAP).
  • Offer advocacy and support, helping clients build self-sufficiency and strengthen their social networks.
  • Maintain accurate and timely documentation, ensuring compliance with agency policies and regulations.

Qualifications

Qualifications:

  • Lived experience in mental health recovery and wellness. Must meet ODMHSAS Peer Recovery Support Specialist criteria.
  • Bachelor’s degree or 3+ years’ experience in the community mental health field preferred.
  • Dual PRSS/CMI/CMII certification is a plus!
  • Current Peer Recovery Support Specialist and Wellness Coach certifications, with a commitment to ongoing education.
  • Certification as a WRAP facilitator is highly desirable.
  • Proficiency in MS Word, general computer skills, and excellent communication abilities.
  • Flexibility to work in diverse community settings, with reliable transportation and valid auto insurance (business purposes required).
  • Current CPR, First Aid, and Therapeutic Options certifications (or ability to obtain within 30 days).

This is a community-based role requiring travel and a high degree of autonomy. As an essential worker, you may occasionally work holidays, weekends, or during office closures.
