About the Role

Carolina Outreach is looking for a compassionate, driven, and clinically skilled ACT Team Lead to join our dedicated team in Smithfiled, NC. As part of the ncgCARE network, we’re committed to transforming lives through evidence‑based behavioral health services — and we need a strong leader to help guide that mission on the ground.

In this full‑time role, you'll lead a passionate multidisciplinary team delivering Assertive Community Treatment (ACT) services. You'll balance administrative oversight and direct client care (approximately 60/40), playing a key role in shaping team culture, maintaining clinical excellence, and ensuring every individual we serve receives the person‑centered support they deserve.

You’ll step into a role that blends strategic leadership with hands‑on care — offering supervision, consultation, and guidance to your team, while also working directly with clients to support their journey toward stability and independence. From crisis response to long‑term planning, your work will touch every aspect of ACT service delivery.

This is more than a leadership position — it’s a chance to make a daily difference in the lives of individuals living with severe and persistent mental illness. You’ll also have the opportunity to influence service innovation, advocate in the community, and elevate standards of care in partnership with a values‑driven organization.

Team Leadership and Supervision
  • Provide clinical and administrative supervision for the ACT team, ensuring adherence to program standards.
  • Conduct individual clinical supervision and case consultations for team members.
  • Respond to crisis situations with staff and act as a back‑up for client needs as required.
Direct Client Services
  • Deliver services such as crisis assessment, symptom management, individual counseling, and psychotherapy.
  • Provide substance abuse treatment, daily living support, and social or leisure activity services to clients.
  • Coordinate education, support, and consultation for clients’ families and natural supports.
Program Oversight and Standards
  • Ensure compliance with the latest National Program Standards for ACT Teams, as defined by the National Alliance for the Mentally Ill or the U.S. Department of Health and Human Services.
  • Oversee both internal and external services, advocate for clients in the community, and collaborate with families and natural supports.
Planning and Evaluation
  • Lead the development and evaluation of person‑centered plans in collaboration with the client and the team.
  • Monitor and ensure services are appropriate, effective, and aligned with client needs.

This description outlines the general scope and level of work for this role. It is not intended to include every duty or responsibility that may be required of employees in this position.

Compensation

This role is compensated on a salaried basis, providing consistent earnings aligned with the responsibilities and expectations of the position. Compensation: $61,449 - $83,441. Placement within the range is based on licensure level, education, and experience.

What You Can Expect

Responsibilities
  • Maintain accurate and timely clinical documentation
  • Collaborate with care teams as needed
