Job summary

ncgCARE is seeking a Peer Support Specialist to coordinate services for clients in Supply, NC. The role requires CPSS certification, personal experience with recovery, and a strong ability to advocate and communicate with teams and clients.

Responsibilities

Service Coordination:

  • Collaborate with the ACT team leader to monitor clients’ clinical status and responses to treatment.
  • Assist in identifying, planning, scheduling, and implementing services tailored to each client’s needs.

Client Advocacy and Support:

  • Act as an advocate for clients in the community, supporting their access to necessary resources and opportunities.
  • Assist in the development of transition plans when clients move in or out of services.

Documentation and Compliance:

  • Maintain accurate records, including treatment notes, goals, and input into person-centered plans.
  • Ensure adherence to the ACT model, as measured by TMACT standards.

Team Collaboration:

  • Provide consultation to the ACT team to promote a person-centered approach to care.
  • Support a culture of respect and understanding for clients’ points of view and preferences.

Availability:

  • Be available to respond to clients and co-workers as needed, including 24/7/365 on-call support.
