Job summary

Envision Health Inc seeks an ACT Licensed Practical Nurse (LPN) to join our Assertive Community Treatment team in Virginia. The LPN provides community-based nursing services, supports medication management, health monitoring, and care coordination for individuals with serious mental illness and co-occurring disorders.

The LPN will administer meds, monitor responses and adverse effects, educate on self-management, and work with the ACT team to implement treatment plans in diverse community

Qualifications

• Virginia Licensed Practical Nurse (LPN) license required.
• Current CPR certification required.
• Minimum one year of nursing experience preferred.
• Behavioral health or community mental health experience preferred.

Responsibilities

• Administer medications as prescribed and within scope of practice.
• Monitor medication effectiveness, side effects, and adherence.
• Educate on medication management and self-care.
• Conduct routine health assessments and monitor vital signs.
• Observe and report changes in health status.
• Provide services in homes, shelters, hospitals, and community locations.
• Accompany individuals to medical appointments when clinically indicated.
• Collaborate with ACT team to implement treatment plans.
• Participate in treatment team meetings and case consultations.
• Document accurately and timely within the electronic health record.

Department: Assertive Community Treatment (ACT)

Reports To: ACT Registered Nurse (RN), ACT Team Lead, and Director of Behavioral Health Services

Position Summary

The ACT Licensed Practical Nurse (LPN) provides community-based nursing services as part of a multidisciplinary Assertive Community Treatment (ACT) Team. The LPN assists individuals with serious mental illness, co-occurring disorders, and complex healthcare needs by providing medication support, health monitoring, education, and coordination of care in community settings.

Essential Duties and Responsibilities

• Administer medications as prescribed and within scope of practice.
• Monitor medication effectiveness, side effects, and adherence.
• Assist with medication education and self-management training.
• Conduct routine health assessments and monitor vital signs.
• Observe and report changes in physical or mental health status.
• Provide services in homes, shelters, hospitals, and community locations.
• Accompany individuals to medical appointments when clinically indicated.
• Collaborate with ACT team members to implement treatment plans.
• Participate in treatment team meetings and case consultations.
• Complete documentation accurately and timely within the electronic health record.

Qualifications

• Graduate of an accredited Practical Nursing Program.
• Current unrestricted Virginia Licensed Practical Nurse (LPN) license.
• Current CPR certification required.
• Minimum one year of nursing experience preferred.
• Behavioral health or community mental health experience preferred.
