Job Summary

The therapist is responsible for planning, providing, and coordinating all aspects of patient care, ensuring high-quality, safe treatment in accordance with clinic policies. The role involves collaboration with healthcare team members and may include responsibilities of the Clinic Director in their absence.

Essential Duties

• Deliver high-quality patient care and assessments
• Perform evaluations, develop and revise individualized care plans
• Communicate effectively with patients, families, and healthcare team members
• Evaluate treatment effectiveness and adjust plans accordingly
• Plan discharge and ensure safety protocols are followed
• Utilize clinical techniques appropriate to the setting
• Participate in quality improvement initiatives
• Complete documentation using AgileRPM
• Travel to other clinics as needed

Qualifications

• Master’s or Doctorate in Physical Therapy
• Valid Physical Therapist license and registration as per state regulations

Physical Demands

• Stand, walk, sit, reach, climb, kneel, crouch, or crawl as needed
• Lift up to 25 pounds regularly and occasionally more than 100 pounds
