Job description

This position is for an Independent Contractor to serve in the Porter Ranch area specifically.

FeldCare Connects, an app-based referral network, is seeking an enthusiastic, self-motivated Armenian-speaking Speech Therapist to deliver quality home healthcare to patients of all ages.

The Speech Therapist in Home Health is responsible for assessing and evaluating patient care needs related to speech and language disorders, and training in communication, swallowing, and cognitive impairments. Based on this assessment, the Speech Pathologist develops a treatment plan, executes interventions to improve the patient’s well-being, and monitors progress effectively.

A Speech Therapist for Home Health must:

• Interpret, plan, establish, and implement treatment plans according to physician referrals.
• Record, review, and report patient responses and any changes in condition.
• Instruct, teach, and monitor patients, families, and healthcare team members regarding speech therapy procedures.
• Make recommendations based on the home setting and individual patient needs.
• Maintain detailed records of each visit and document notes in the medical record.
• Review patient responses to treatment and advise physicians on the continuation or adjustment of therapy.

Qualifications:

• Ability and enthusiasm to work with patients of all ages.
• Speech Therapist license and registration by the state.
• Completion of an accredited Speech Therapy program.
• Certificate of Clinical Competence (CCC) in speech pathology or audiology from the American Speech-Language-Hearing Association.
• Bilingual skills a plus!
