  • First Aid certification within 30 days of employment.
  • CPR certification within 30 days of employment.
  • Preferably bachelor’s degree in related field.
  • Preferably 1 year of SUD experience.

Responsibilities

JOB SUMMARY

HealthRIGHT 360, a nonprofit organization and a family of programs, is committed to providing accessible and comprehensive healthcare services to vulnerable populations. Our mission is to tackle systemic barriers to healthcare and promote health equity for all. We offer a wide range of services, including mental health care, residential and outpatient substance use treatment, and primary health services. Additionally, we provide transitional support for individuals re-entering the community after involvement in the criminal justice system. By integrating physical and behavioral health, we empower individuals to overcome challenges by addressing social determinants of health, fostering resilience, and facilitating recovery.

The Resilient Recovery Program provides youth and adults with substance use and co-occurring mental health services in the form of individual and group counseling and care coordination. This program is committed to working with underserved communities from a variety of backgrounds. We strive to meet the ongoing needs of individuals struggling with mental health issues, substance use disorders, and co-occurring mental health issues.

Care Coordinators are an important part of the San Mateo County Outpatient Program. Utilizing a trauma informed approach, they are responsible for assessing adult and youth client needs and strengths to collaboratively create treatment plans/problem lists with their clients that support their health and recovery. The care coordinator/counselor will provide individual and group counseling to adults and youth; deliver evidence-based curricula in accordance with program specific requirements. Care Coordinators assists clients in navigating systems of care while providing a supportive treatment environment.

May work weekends and holidays as needed.

KEY RESPONSIBILITIES
Individual Treatment Responsibilities
  • Facilitates intakes/orientations, comprehensive assessments, individualized treatment planning, crisis, collateral, case management and individual counseling sessions with clients.
  • Offers clinical support and counseling to assist clients in meeting their treatment goals including providing feedback, setting boundaries, and celebrating successes.
  • Proactively links clients to both internal and external resources based on their treatment needs and follows up on the progress/status.
Treatment Setting Responsibilities
  • Facilitates group counseling sessions and teaches classes on various treatment-related topics.
  • Conducts screenings and intake assessments to determine client’s eligibility and ASAM level of care placement.
  • Performs crisis intervention and communicates with treatment team as unforeseen situations arise.
  • Completes and attends required training and meetings.
  • Coordinate discharge planning in collaboration with the rest of the treatment team.
  • Comply with certifying agency requirements for registration/certification renewal.
  • Plans and facilitates client celebrations and special events.
  • Will work one weekend day based on rotating schedule to facilitate the scheduled weekend groups.
  • Will work at agency’s main office and in addition to co-location sites which may include schools, community health centers, in-home, and partner agencies.
Documentation Responsibilities
  • Collaborates with each caseload client and other available internal and external resources to develop/maintain treatment plans/problem lists, transition/discharge plans, progress notes and appropriate updates in support of the health and recovery needs of the client.
  • Properly documents all individual and group counseling sessions, intake and assessments, completes discharge paperwork and referrals in a timely manner as required by DMC and county.
  • Maintains accurate records by entering documentation into various electronic systems for all caseload clients in accordance with guidelines established by HealthRIGHT 360 to satisfy internal and external evaluating requirements.
  • Will participate in Peer Chart/ Utilization Reviews for preparation of program audits. Will adhere to documentation policies and procedures set forth by Compliance Department.
  • Be responsive and proactive in all correspondences with referring parties such as behavioral health providers, probation, social workers, and the Quality Improvement Dept. via treatment status reports, phone calls, faxes, etc.
  • Responds to records updates and corrections in a timely manner to ensure documentation compliance and accuracy.
  • Will assist in providing written progress reports/narratives to Clinical Program Manager or Directors as requested.
Productivity and Coverage Responsibilities
  • Must meet productivity standards as set forth by the organization, the minimum being 50% productivity.
  • Ensure coverage during absences or unplanned events by coordinating with team members and management.
  • Adjusts workload as necessary to ensure uninterrupted service delivery, including stepping in to provide support during peak demand times or staff shortages.
  • Adheres to the program’s scheduling protocols to maintain appropriate client-to-provider ratios.

QUALIFICATIONS
Education, Certification, and Experience
  • High School diploma or equivalent OR associate degree in a related field/
  • Must be registered as an Alcohol and Other Drug Counselor or as an SUD Counselor with CA approved certifying agency (CCAPP, CADTP, or CAADE).
  • First Aid Certified within 30 days of employment.
  • CPR Certified within 30 days of employment.
  • Preferably bachelor’s degree in related field.
  • Preferably 1 year of SUD experience.
  • Relevant experience working in outpatient settings preferred.
  • Application of Gender Responsive and Trauma Informed principles and interventions.
Knowledge
  • Culturally responsive and able to work with a diverse population.
  • Strong proficiency with Microsoft Office applications, specifically Word, Outlook, and Internet applications.
  • Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic self-sufficiency.
  • Knowledge of the principles of client-centered, strength-based practice (including the principles of the recovery model and trauma-informed practice).
  • Understanding of harm reduction model.
  • Understanding and experience working with recovering individuals and their impacted family members.
  • Knowledge of cultural stigmas of counseling and treatment services.
  • Knowledge of principles and techniques of social work, case management, crisis intervention, client centered counseling, and group facilitation.
  • Knowledge of child development.
  • Knowledge of and experience with substance abuse services in the county.
  • Proficient with clinical documentation and EHR systems.
  • Knowledge and experience with Medi-CAL documentation standards preferred.
  • Knowledge of mental health recovery and evidence-based practices preferred.
