Job summary

Community Health and Wellness, LLC is seeking qualified Aquatic Therapists to deliver one-on-one aquatic therapy services across Texas, including San Antonio. You will assess needs, set tailored goals, document progress, and coordinate with care teams to support individuals with disabilities.

Travel to clients’ homes is required; candidates must have reliable transportation, HIPAA-compliant devices, and hold relevant certifications. Background checks will be conducted.

Qualifications

• Must be CPR/AED/FIRST AID certified (American Red Cross or American Heart Association).
• CTRS or TRS/TXC or LMT certification is required.
• HIPAA compliance requires access to a private laptop/desktop.
• Background and reference checks will be conducted.

Responsibilities

• Provide one-on-one Aquatic Therapy to children and adolescents with intellectual or physical disabilities and behavioral/mental health diagnoses.

• Document on each session.

Job Description:

• Provide one-on-one Aquatic Therapy to children and adolescents with intellectual disabilities, physical disabilities, and behavioral/mental health diagnoses
• Create tailored goals to meet the needs of individual clients
• Document on each session
• Communicate with other team members and service providers
• Turn in timely documentation including weekly invoicing, progress notes, and related paperwork

Essential Duties and Responsibilities

• Must be able to travel to current or prospective clients’ homes
• Must be able and willing to transport client(s) during client visits
• Must have access to a private personal laptop or desktop computer for HIPAA compliance
• Track SMART goals and progress through weekly documentation using Microsoft Word, Excel, and Adobe Reader
• Implement patient care plans through home and community goal-related activities
• Able to work independently, manage one’s own time and the time of client(s)

All therapists must have the following:

• Resume
• CTRS or TRS/TXC or LMT certified
• CPR/AED/FIRST AID Certification (American Red Cross or American Heart Association only)
• Basic Water Rescue and/or Lifeguard Training
• Social Security Card
• Driver's License
• Auto Insurance (with 100/300/100 coverage minimum)
• Professional Liability Insurance (with $1 million/$3 million coverage limit)
