Job summary

The Agency HRA/DEPT OF SOCIAL SERVICES in New York City is seeking an APS Social Worker to provide psychosocial services to adults and families within the Adult Protective Services program.

You will conduct assessments, link clients to services, coordinate with hospitals and clinics, and help prepare treatment plans and possible Article 81 orders while maintaining licensure and residency requirements.

Qualifications

• Master’s Degree in Social Work from an accredited school.
• Valid LMSW or LCSW license issued by NYSED within one year of appointment.
• New York City residency generally required within 90 days of appointment.

Responsibilities

• Provide psychosocial services to individual clients/patients, or to families and/or other groups of adults/children.
• Apply social work skills to consult with caseworkers and supervisors on complex cases.
• Complete psychosocial assessments for housing applications and other services.
• Link clients with public and private programs for services.
• Collaborate with social workers in hospitals, clinics, and other medical facilities.
• Assist in preparation of treatment plans and legal documents for Article 81 court orders.
• Interview and assess clients to determine ability to function in the community.
• Evaluate the quality and appropriateness of referrals and care.
