Job summary

HealthFirst Bluegrass is seeking a compassionate, mission-driven APRN to provide high-quality primary care in our School-Based Health Clinics, focusing on children and adolescents while supporting patients of all ages within our community health system.

As part of our team, you'll help remove barriers to care, perform preventive, acute, and chronic care, educate patients and families on wellness, and collaborate with school personnel and healthcare partners to expand access.

Qualifications

• Active Kentucky APRN license and NP certification are required.
• CPR certification required.
• Experience in pediatric, family practice, community health, or school-based care preferred.

Responsibilities

• Provide preventive, acute, and chronic care services.
• Perform assessments, diagnoses, and treatment planning.
• Educate patients and families on wellness and self-care.
• Collaborate with school personnel, community partners, and healthcare teams.

As part of our team, you'll help remove barriers to care, improve student health outcomes, and support families through accessible, patient-centered healthcare.
