Job summary

Concord Hospital seeks an Advanced Practitioner to support the Hospitalist Program. The role involves acute inpatient care, initial encounters, daily rounding, and coordinating dispositions under physician supervision.

You will provide preventive care, patient education, and collaborate with the care team to ensure safe, effective hospitalization. The ideal candidate will have completion of an accredited PA or APN program, NH licensure, active ACLS, and NCCPA/AANP/ANCC certification.

Qualifications

• Completion of an Accredited Physician Assistant Program or Advanced Practice Nursing Program.
• Provider experience required.
• Current ACLS certification.
• Current license to practice in state of New Hampshire.
• Certified by NCCPA, AANP, or ANCC.

Responsibilities

• Obtains patient health history, performs physical examination, develops a differential diagnosis, and documents encounters.
• Orders, interprets, and follows up on diagnostic tests.
• Diagnoses acute and chronic medical problems within the scope of practice; requests specialty consultations as needed.
• Prescribes and manages medications within practice standards.
• Provides patient education related to preventive care for hospitalized patients.

Summary

The Advanced Practitioner for the Hospitalist Program provides acute care services to patients in the inpatient setting; including initial encounters and dispositions, daily patient rounding, consultations, preventive care and patient education. Patient care is provided under the supervision of the supervising physician.

Education

Completion of an Accredited Physician Assistant Program or Advanced Practice Nursing Program.

Certification, Registration & Licensure

• Current license to practice in state of New Hampshire
• Current DEA License
• Certified by the National Commission on Certification of Physician Assistants, American Academy of Nurse Practitioners, or American Nurses Credentialing Center
• Current ACLS certification

Responsibilities

• Obtains patient health history, performs physical examination, develops a relevant differential diagnosis, and documents encounters appropriately.
• Orders, interprets, and follows up on diagnostic tests.
• Diagnoses acute and chronic medical problems within the scope of practice, or formulates a comprehensive differential diagnosis and takes the necessary steps to obtain a diagnosis, including appropriate specialty consultations.
• Prescribes and manages medications within the practice standards.
• Provides individualized patient education related to preventive care according to guidelines and relative to the care of the hospitalized patient.
• The Advanced Practitioner documents in the medical record according to the department standards.
• Provides effective communication with the multidisciplinary members of the patients care team, including consultants and specialists.
• Performs medication reconciliation upon admission and discharge to ensure medication safety, eliminate interactions, and avoid omissions upon discharge.
